Removing the filler in a nutshell

Filler removal is a popular procedure. There are many different reasons why people seek removal. A previous filler treatment may have been unsuccessful, the filler may have migrated or encapsulated, or too much filler may have been injected. An exceptionally large amount of filler is removed from the lips: naturalness, elegance, and subtlety are major trends, and overly plump lips may now be perceived as embarrassingly tacky.

The availability of hyaluronidase is part of the emergency protocol at a professional aesthetic clinic. Any complications arising from filler treatments can be resolved quickly and effectively with hyaluronidase.

Removal of Filler: Dr. Laura Juopperi and surgeon Männistö in Espoo

Filler removal is almost always performed using hyaluronidase. Hyaluronidase is an enzyme that breaks down hyaluronic acid.

Encapsulated lip fillers can sometimes be removed with a needle. This is only possible if the filler to be removed forms a clearly defined, pea-sized lump.

The removal of the filler begins with numbing the target area. Hyaluronidase is then injected into the area.

Hyaluronidase begins to break down the filler immediately, but the final result can only be assessed after two weeks. It is not always possible to remove all of the filler in a single treatment session.

If you want to have another filler treatment in the area where the filler was removed, you should wait about four weeks.

Filler removal is a popular procedure

Filler removal is suitable for adult women and men. Most often, people seek this treatment when the results of the filler procedure do not meet their expectations or when unevenness, swelling, or other undesirable changes have developed in the treated area.

Treatment topics:

  • an uneven or unnatural-looking result
  • excessive filling or imbalance in facial features
  • migration of the filler material away from its original location
  • prolonged swelling or tissue reaction
  • the need to restore the face to its original shape

If the issue is minor asymmetry or a small area that needs finishing, it is not always necessary to remove all of the filler.

What the treatment does and what it doesn't do

  • The filler is removed using hyaluronidase, which breaks down hyaluronic acid in the tissue and allows for the relatively rapid breakdown of the filler’s cross-links.
  • The goal is to restore the tissues to as close to their natural state as possible.
  • The treatment is effective only on pure hyaluronic acid-based fillers.
  • Hyaluronidase does not break down semi-permanent lactic acid or calcium derivatives, nor does it break down any impurities that may be present in hyaluronic acid products.

How is the removal of the filler proceeding?

The removal of fillers begins with a relaxed consultation and planning of the procedure. Hyaluronidase injections may cause moderate discomfort. The sensation is often described as a prickling or stinging feeling, and it may feel more intense than the actual filler treatment, especially in sensitive areas such as the lips.

Consultation

The process of removing fillers begins with a consultation. You can book a free consultation appointment even while you’re still considering removal. During the consultation, you can ask questions, discuss your situation, and meet our professionals. The consultation also determines whether the filler to be removed is definitely a hyaluronic acid product that can be broken down with hyaluronidase.

A separate consultation visit is not required. We consult with all clients before beginning the removal process in any case, and if it is determined during the consultation that the treatment cannot be performed for one reason or another, the visit automatically becomes a free consultation.

If you are familiar with aesthetic injection treatments, you can book an appointment directly for filler removal. In cases where you are unsure, we recommend scheduling a separate consultation before the procedure.

Hyaluronidase injections

Following the consultation, the procedure will begin. The skin in the area to be removed will be cleaned. If necessary, the area will be numbed.

Hyaluronidase is injected into the target area in small doses. The sensation is often described as a prickling, stinging, or burning feeling. Removing the filler may be slightly more painful than the actual filler treatment, especially in sensitive areas such as the lips.

After removing the filler

After the filler is removed, the skin is cleaned and the injection sites are checked. Small injection marks may remain on the skin. These fade quite quickly.

Swelling, redness, and tenderness often develop in the treated area. Bruising is also possible. These side effects are common and temporary. However, they may last for several days.

Unless the swelling bothers you, you can return to your normal routine after the procedure. However, you should avoid touching the treated area, strenuous exercise, alcohol, and saunas for at least the first 24 hours.

All aftercare instructions are reviewed verbally during the appointment. The client also receives written aftercare instructions.

When will the results be available?

The removal of the filler begins to take effect quickly as hyaluronidase breaks down the filler in the tissue. Changes may be visible within 24–48 hours, but the final result can only be assessed once the area has healed and the swelling has completely subsided.

  • The first changes may become apparent within 1–2 days
  • The final outcome will be assessed once the swelling has subsided a few weeks after the procedure

“When removing filler, the change may be visible quickly, but swelling can obscure the final result in the early stages. That is why the situation is often assessed after a few days or weeks.” (Surgeon Ville Männistö)

Allergy test before hyaluronidase injections

  • Allergy risk: In rare cases, hyaluronidase may cause a severe allergic reaction.
  • Allergy test: It is advisable—and sometimes even necessary—to undergo an allergy test before removing the filler.
  • Performing the test: A small amount of hyaluronidase is injected into the skin (usually in the arm). The reaction is monitored for about 20 minutes.
  • When is the test performed: The test is performed before the actual procedure, unless it is an emergency. It is particularly important if this is the first time the filler is being removed.
  • Follow-up: After the procedure, the patient is monitored in the waiting room for 20–60 minutes.
  • New filler treatment: A new filler treatment is recommended only once the tissue has healed, usually no sooner than 2 weeks after the previous treatment, and sometimes later.

During the consultation, we always review any allergies and the safety of the treatment before planning the procedure.

Frequently Asked Questions About Filler Removal

What does "removing filler" mean?

Filler removal involves breaking down a hyaluronic acid-based filler using an enzyme called hyaluronidase.

Why do people want to remove the filler?

Removal is performed, for example, if the final result does not meet expectations, the filler has shifted to the wrong spot, or unevenness has formed in the area.

This rare vascular complication is treated by promptly removing the filler.

How is the filler removed?

Hyaluronidase is injected directly into the area where the filler is located. The enzyme begins to break down the filler rapidly.

Does the filler get removed?

The removal may feel like a prickling, stinging, or burning sensation. Some people find it even more intense than the original filler treatment, especially in sensitive areas such as the lips.

How quickly does the filler disappear?

The effects begin quickly, and changes may be visible within 24 to 48 hours. The final outcome is assessed once the swelling has subsided.

Will more than one treatment session be needed?

Sometimes a single treatment is enough, but multiple treatments may be necessary if there is a large amount of filler, if it has a dense texture, or if it is in multiple layers.

What are the side effects associated with filler removal?

The most common symptoms are swelling, redness, tenderness, and bruising. These symptoms are usually temporary.

Can removing the filler cause an allergic reaction?

Yes, hyaluronidase can cause an allergic reaction in rare cases. For this reason, an allergy test is often performed before removing the filler.

Can I get a new filler after the removal?

It is not recommended to apply a new filler right away. The area should be allowed to heal, and the next treatment should be performed at a later date.

Can the removal of the filler fail?

It is not always possible to remove all of the filler material in a single session. The final result is often not assessed until several days or weeks later.

Can I go to work after the procedure?

Yes, you can, but swelling or bruising may be visible, so many people schedule their treatment for the weekend, for example.

Is it safe to remove the filler?

When performed by a professional, the treatment is safe, but as with all injection treatments, there are risks involved, which will be discussed before the procedure.
