A well-defined jawline gives the face a sense of poise. On the other hand, even a small change in the lower face can make one’s features appear tired and aged. Most often, changes in appearance develop gradually and go unnoticed. This is also how “hamster cheeks” develop.

Many people first notice their "hamster cheeks" in photographs. The oval shape of the face has softened, and the sharp jawline has given way to a rounder or even square appearance.

Most surprisingly, the shape of the face shape often has a greater impact on one’s appearance than individual wrinkles.

Hamster cheeks are one of the most common—and one of the most misunderstood—signs of aging in the lower face. Only rarely are they caused solely by sagging skin. Most often, the appearance of these bags is due to many simultaneous changes that have developed gradually in different parts of the face and in different layers of tissue.

Fat pads shift downward, and collagen—which maintains the skin’s firmness—decreases. Changes also occur in muscle function and the jawbone. As the skin and the tissues beneath it give way to gravity, the jawline eventually begins to sag.

Hamster bags are a multifaceted age-related change. They cannot be prevented or treated by focusing on any single factor.

"Hamster bags " – a descriptive but crude term

"Hamster cheeks" is a harsh but apt description. When soft bulges form on both sides of the jaw, many people feel that the lower face begins to resemble a hamster's cheeks stuffed with food.

This isn’t a medical diagnosis but rather a term that has become part of our everyday language. Aesthetic medicine professionals may use more technical terms like “tissue laxity” or “softening of the jawline,” but clients who come in for a consultation usually refer to them simply as “hamster cheeks.”

Hamster bags or something else entirely?

Not all changes to the jawline mean "hamster cheeks." Here are the most common differences:

🐹 Hamster Bags

Bulges appear on both sides of the tip of the chin, usually slightly below the corners of the mouth. They blur the contours of the face and make it look heavier or more tired than before.

👇 Double chin

A double chin forms under the chin, rather than along the jawline. It is often associated with fatty tissue, sagging skin, and genetic factors. Like hamster cheeks, a double chin is a cosmetic issue, but it is usually treated with different methods than those used for hamster cheeks that form along the jawline.

😊 Round cheeks

Round cheeks are a completely natural and youthful facial feature. They do not alter the shape of the jawline in the same way that hamster cheeks do. Round cheeks may even provide more support to the lower face than a very narrow midface.

Hamster Cheeks: More Than Just Sagging Skin

Under-eye bags are a rather significant cosmetic issue: almost always, they affect the overall appearance more than small, isolated fine lines here and there. It’s also difficult to prevent under-eye bags.

Hamster cheeks are often thought of as nothing more than loose skin or excess fat. In reality, the underlying causes and chains of events are much more complex.

Think of a single row on a bookshelf. When the shelf is level, the books in that row stay neatly balanced and in place. But when the shelf gives way after years of wear and tear, the books shift as well—even though there aren’t any more of them than there were before.

A similar phenomenon occurs in the face: the supporting structures weaken, and the soft tissues begin to shift. The proportions of the face change, and suddenly looking at your own reflection in the mirror starts to cause confusion.

The epidermis takes the first hits

The epidermis is the outermost layer of the skin. It is the first place where signs of aging begin to appear. The good news is that age-related changes in the epidermis can be addressed with skincare products. In most cases, a diligent, simple at-home skincare routine is sufficient.

Dermis: The skin's master builders are slacking off

The epidermis is the outer layer of the skin, while the dermis is its supporting structure. The extracellular matrix of the dermis contains fibroblasts, whose vital role is to maintain and regenerate the skin’s stores of collagen, elastin, and hyaluronic acid.

As we age, fibroblast activity slows down. As a result, the levels of collagen, elastin, and hyaluronic acid also decrease. The skin’s support structure weakens. The skin becomes thinner and can no longer bounce back to its original shape after being stretched or subjected to stress.

Changes in the dermis alone do not cause jowls, but a sagging dermis gives in to gravity sooner than a healthy dermis.

Prevent age-related changes in the dermis

Ligaments hold the face together—or do they?

If you were to compare the face to a tent, the ligaments would be its guy ropes. They connect the skin and soft tissues to the deeper structures and keep the different layers of the face in their proper places.

For a long time, it was thought that hamster pouches formed when these “supporting ligaments” loosened over the years.

The concept is easy to understand: when the tent guy ropes give way, the fabric starts to sag.

However, the scientific truth appears to be more complex.

Anatomical studies in recent years suggest that ligaments do not necessarily stretch significantly with age. Instead, a lot happens around them. Fat pads shrink and shift, the skin loses its elasticity, the SMAS layer changes, and the support provided by the skeleton decreases. The end result, however, is the same: the jawline sags and under-eye bags begin to appear.

WHAT DO WE KNOW FOR SURE?

SMAS holds the layers of the face together

SMAS sounds like a complex medical term. The idea behind it, however, is surprisingly simple. SMAS is the face’s own support network, which connects the muscles and soft tissues beneath the skin to one another. When you smile, laugh, or furrow your brow, the SMAS moves along with the tissues.

In young faces, the SMAS keeps the tissues firmly in place. As we age, the SMAS changes. Together with the skin, fat pads, and other supporting structures, it weakens and sags. This change inevitably becomes visible on the outside. The cheekbones become flatter, and the contours of the jawline soften. The hamster bags become more pronounced.

SMAS IN A NUTSHELL

DON'T FALL FOR IT!

SMAS is just one piece of the puzzle. Changes in SMAS do not fully explain the "hamster pouches," and the "hamster pouches" cannot be eliminated simply by tightening the SMAS structure.

"If there is a significant amount of excess tissue associated with hamster cheeks, only a plastic surgeon can correct the condition," notes Dr. Laura Juopperi.

The platysma and DAO muscles pull the lower face downward

Over the years, the muscles also change. Of particular importance for hamster cheeks are the broad platysma muscle in the neck and the DAO muscles (depressor anguli oris), which pull the corners of the mouth downward. The platysma and DAO muscles belong to a group known as depressors, whose function is to pull the corners of the mouth and the lower cheeks downward.

DID YOU KNOW?

The platysma and DAO are not entirely separate muscles. Anatomical studies have shown that their muscle fibers can cross and interconnect in the area of the corners of the mouth and the lower jaw. For this reason, they often function as a single unit, pulling the corners of the mouth and the lower face downward.

For this reason, in aesthetic medicine, the platysma and DAO muscles are often evaluated and treated together, rather than as separate structures.

The jawbone also ages

When discussing facial aging, attention is usually focused on the skin, wrinkles, and sagging tissues.

Even the facial bones change.

The jawbone doesn’t disappear overnight, but its shape gradually changes. Bone tissue regenerates throughout life, and as we age, bone mass decreases. In particular, the front part of the lower jaw and the jawline may lose their prominent shape. You rarely notice this change in the mirror. You only realize it years later when looking through old photographs.

The soft tissues of the face need a firm foundation beneath them. As the support provided by the bones diminishes, the skin, fat pads, ligaments, and muscles also lose some of their foundation.

Rapid weight loss comes as a surprise

Many people are happy about losing weight, but from the perspective of the face, rapid weight loss is a double-edged sword. When fat tissue decreases rapidly, the supporting structures of the face may not have time to adapt to the change. The so-called “Ozempic face” is therefore not caused by the medication but by rapid weight loss.

On social media, you’ll find an endless number of tips—from facial exercises to taping and gua sha massage. Some of these tips may help reduce puffiness or temporarily make your skin feel firmer.

Let's see what we actually know from the research—and where the evidence is still limited.

Facial exercises are a topic of debate

Facial exercises and facial yoga have become a phenomenon on social media in recent years. Some swear by them, while others see them as little more than a fad and nonsense. The truth lies somewhere in between. Facial muscles can be exercised in the same way as any other muscles. At the same time, it’s important to remember that under-eye bags are much more than just a muscle problem.

What is known?

  • The facial muscles respond to exercise in the same way as other muscles in the body.
  • Regular facial exercises can increase the activity and firmness of certain facial muscles.
  • Moving your face can temporarily increase blood flow and brighten your skin.
  • Facial exercises do not move sagging fat deposits back to their original locations.
  • Exercise also does not repair the skin or affect age-related changes in the ligaments, the SMAS layer, or the bones.

What is not known?

  • Can facial exercises slow the development of jowls in the long term?
  • Which exercises are most effective specifically for the jawline area?
  • How often should you exercise to achieve lasting results?
  • Who benefits most from facial exercises, and for whom do they offer no significant benefits?

A few small clinical studies on facial exercises have been published, reporting mild improvements, particularly in midface fullness and perceived youthfulness. However, the evidence remains limited, and studies have not shown that facial exercises eliminate or significantly reduce under-eye bags. Based on current research evidence, facial exercises can be considered a safe supplement to support skin and facial well-being, but their effects on under-eye bags are highly uncertain. Alam et al. (2018); Van Borsel et al. (2014).

Face masking is trending on social media

Face taping has quickly become a hit on TikTok and Instagram. The tapes are said to lift the jawline, smooth out fine lines, and prevent the formation of “hamster pouches” overnight. These relatively inexpensive tapes are easy to try at home, but there is very little scientific evidence to support their claims.

WHAT IS KNOWN?

Facial taping can temporarily reduce skin sagging and make the skin feel firmer immediately after the tape is removed. However, the tape does not produce lasting results.

IN THE LIGHT OF RESEARCH

There is no high-quality clinical evidence regarding the effects of facial taping on hamster pouches. According to current understanding, taping cannot alter the position of fat pads, strengthen ligaments, or correct other underlying anatomical changes associated with hamster bags. Therefore, any potential benefits are likely to be temporary.

Gua sha helps you relax

Gua sha is a massage technique based on traditional Chinese medicine that has also found its way into skincare routines. It can feel pleasant and reduce facial swelling. However, its effectiveness in treating under-eye bags is limited.

There are virtually no studies on the effects of gua sha on hamster cheeks.
Current research mainly points to a reduction in swelling and a temporary increase in blood flow. There is no scientific evidence of lasting effects on facial features or the shape of the jawline.

  • Massage can reduce facial swelling and fluid retention.
  • Many people find that after a facial treatment, their faces look rested and refreshed.

The power serum doesn't reach the sites where hamster pouches form

Regular skincare makes your skin healthy, smooth, and beautiful, but even the most effective skincare routine has its limits. Hamster bags form mainly in the dermis and in the subcutaneous soft tissues. Creams and serums cannot penetrate these areas, nor are they meant to penetrate that deeply.

Retinoids—retinol, retinyl, and tretinoin—can increase collagen and elastin production in the dermis, but even these cannot lift sagging fat pads, tighten ligaments, or alter the structure of the SMAS layer.

At-home skincare with all the extras

A healthy lifestyle won't eliminate hamster cheeks, but it can slow their development.

High-quality skincare and a generally healthy, relaxed lifestyle are excellent ways to prevent all signs of aging, but they cannot on their own eliminate under-eye bags that have already formed.

The treatments complement each other

A good example is a person whose “hamster cheeks” are caused by both sagging skin and a loss of volume in the midface. If the skin is tightened with a laser but the deeper tissue support is not restored, the jawline may still appear heavy. On the other hand, fillers alone do not correct sagging of the skin’s surface layer.

The same principle applies to muscles. In some people, the platysma muscle pulls the jawline downward more strongly than usual. In such cases, Botox can balance muscle activity, but it does not increase collagen production or reverse age-related changes in the dermis.

A single treatment addresses one part of the whole.

Treatment progresses step by step

For many patients, the primary goal is to improve the condition and appearance of their skin. Once the skin is stronger and more elastic than before, subsequent treatments can also yield good results.

Once the skin has been restored, the next step is to reduce the effect of the downward-pulling muscles or to provide support for aging tissues. Only after this is done is it determined whether mechanical lifting—for example, using sutures—is necessary.

Not every treatment option is necessary for everyone. In a relatively young patient, slight sagging of the cheek area may be related almost exclusively to muscle function or the onset of collagen loss.
